PROGRAM DESIGN 

The program is designed to allow the fellows to see patients in a variety of settings.  This will allow the fellow to gain experience with treatment and treatment planning based on the patient’s needs. There are 6 blocks where fellows will spend their time. In the Medical stabilization/detox rotation, the fellows will rotate on an inpatient unit to lean withdrawal and medical management for those who have substance use disorders who require this intensity level to be able to safely detoxify from the substance of abuse. This is the rotation that will provide exposure to medically compromised individuals with substance use disorder. In the Residential Treatment Program rotation, fellows will treat persons with Substance use disorder who need a more structured, intensive level of service.  In this rotation, residents tend to stay in treatment for a longer period of time, allowing fellows to observe progress towards treatment goals as well as emphasis on psychosocial stressors which may be contributing to a person’s inability to remain sober. In the Intensive Outpatient Treatment rotation, fellows will have the opportunity to engage with those persons with Substance use disorder who require a less intensive level of service than residential treatment, but a more intensive level of treatment than traditional outpatient treatment. This rotation will emphasize skills needed to maintain sobriety, medication management, if appropriate, and the use of group therapy as a treatment modality. Fellows will be able to interact with those persons who may also be learning to cope with outside stressors as patients enrolled in IOP, live in alternative settings, such as home or sober community. In the Psychiatry Dual Diagnosis rotation, fellows will be able to evaluate, diagnose and treat those with co-occurring Substance use disorder and a mental health disorder. The fellow will observe how co-occurring disorders with proper identification and treatment can influence prognosis. Outpatient Clinic runs throughout the year. Fellows will have the opportunity to use evidence based practices for the treatment of Substance Use Disorders, in particular Opioid use disorder. This rotation provides the opportunity to manage those who require a less intensive level of care, and who are able to participate in outpatient care. Fellows can follow patients over a longer time frame. Fellows will also learn to adjust and modify treatments based on patient circumstances. Lastly, Fellows can take electives and can choose any rotation which is relevant to the fellowship, such as Emergency medicine, Physical therapy, Pain management, as well as also take opportunity during this time for research or quality improvement project development.
